How to Become a Medical Social Worker
How Do You Become a Medical Social Worker?
Here is how to become a medical social worker: earn a CSWE-accredited Bachelor of Social Work (BSW), get a field placement in a health setting, then finish a Master of Social Work (MSW), which advanced standing can cut to about one year (32 credit hours at UCF, for example). A BSW alone qualifies you for some federally defined health-care roles, such as hospice social worker after a year of health-care experience. Healthcare social workers earn a median $67,880 (BLS, May 2025).

What a Medical Social Worker Does
A medical social worker helps patients and families deal with the practical and emotional fallout of an illness, an injury or a new diagnosis. The Bureau of Labor Statistics (BLS) files the job under healthcare social workers, who “help clients understand their diagnosis and adjust their lifestyle, housing, or healthcare.” In a hospital, that often means discharge planning: BLS gives the example of helping people “transition from the hospital to their homes and communities.”
On a typical shift you might:
- assess a patient’s needs, strengths and support network soon after admission;
- explain what a diagnosis means for work, money and caregiving;
- line up home care, equipment, rehab beds or transportation before discharge;
- respond to a crisis, such as suspected abuse or a mental health emergency;
- follow up to make sure the plan held, and keep the case record current.
BLS notes that some healthcare social workers specialize in medical social work, while others focus on geriatric social work or hospice and palliative care. The skills overlap, so a health-focused BSW prepares you for all three.
How to Become a Medical Social Worker, Start to Finish
Medical social work is an MSW-route career: federal rules reserve the home health social worker title for master’s graduates, and any clinical work needs an MSW and a clinical license. The BSW is still the fastest way in, because it shortens the MSW and qualifies you for health-care roles while you finish. The order matters here, so these are the steps in sequence:
- Earn a CSWE-accredited BSW. Choose a program with a health track or strong hospital placements. Only a BSW from a CSWE-accredited program qualifies you for advanced standing, according to CSWE.
- Do your field practicum in a health setting. CSWE requires at least 400 field hours in a BSW. Ask for a hospital, hospice, dialysis or home health placement.
- Get your state’s bachelor’s credential if it offers one. 42 of 52 jurisdictions issue a bachelor’s-level social work license or registration.
- Apply for advanced standing in an MSW. BSW graduates can skip part of the MSW. At Marshall University that means 30 or 36 hours in one calendar year.
- Add a clinical license if you want to treat mental health conditions. BLS says that takes an MSW, supervised clinical experience after graduation and a clinical exam.
Use the BSW to MSW route finder to see which schools offer both degrees, or read the full BSW to MSW guide.
Health-Care Jobs a BSW Can Fill Before the MSW
Federal Medicare rules spell out who can hold social work roles in facilities that bill Medicare, and they show exactly where a BSW counts. Each one asks for health-care experience, which is why your practicum placement matters so much.
| Setting | Role | Degree the federal rule requires | Experience |
|---|---|---|---|
| Hospice | Social worker | MSW, or a CSWE-accredited BSW (other related bachelor’s degrees need MSW supervision) | 1 year in a health-care setting |
| Nursing home with more than 120 beds | Full-time qualified social worker | Bachelor’s in social work or a human services field | 1 year supervised, working directly with individuals |
| Home health agency | Social work assistant | Bachelor’s in social work or a related field, under a qualified social worker | At least 1 year in a health-care setting |
| Home health agency | Social worker | Master’s or doctorate from a CSWE-accredited school | 1 year in a health-care setting |
The pattern is clear. A BSW opens hospice and nursing home social work, and home health assistant roles, once you have a year of health-care experience. The home health social worker title needs the MSW. Hospital postings vary, so read the ones in your area to see which degree they ask for.
Licensing: Bachelor’s, Master’s and Clinical
Social work licensing belongs to your state. CSWE puts it plainly: licensure after the degree “is under the authority of a designated board within each individual state government.”
What that means for you:
- Bachelor’s level. 42 of 52 jurisdictions issue a bachelor’s-level credential, under titles such as LBSW, LSW, RSW and LSWA, and 31 of them name the ASWB Bachelors exam. California, Colorado, Connecticut, Florida, Georgia, Illinois, New York, Rhode Island, Vermont and Washington issue none.
- Clinical level. All states license clinical social workers. You need an MSW, supervised clinical experience after graduation and a passing score on a clinical exam.
If you plan to work in a big hospital market such as Los Angeles, New York City or Chicago, note that California, New York and Illinois are all on the no-bachelor’s-license list. In those states the MSW carries more weight early on. Check your own state in the BSW license checker or the LBSW license guide.

Florida stands out: three public universities with health tracks, each feeding a one-year or flexible MSW at the same school. If you live there, see all Florida BSW programs.
What These Programs Cost
Federal tuition and fees for a full-time year (NCES, 2023-24) across the 14 programs run from $4,879 at Florida Atlantic University to $60,438, with a median of $8,942. The gap between public and private is wide: the public-school median is $6,399 and the private median is $34,502.
Program-level rates add detail:
- Florida Atlantic lists $203.29 per credit hour for Florida residents in 2026-27 ($799.72 for nonresidents).
- Salish Kootenai College lists $148 per credit for Montana residents in 2025-26, with lower rates for Indian and Indian-descendant students.
- Indiana Wesleyan’s online BSW is $455 per credit hour, and it requires at least 45 transferable credit hours before you start the bachelor core.
Because you will likely pay for an MSW too, the low cost of a public BSW matters twice. Compare total costs in the BSW cost comparison tool.
Medical Social Work Internships During the BSW
Your field practicum is your first medical social work internship, and it is often the deciding line on a hospital application. CSWE requires at least 400 hours in a BSW and 900 in an MSW, and advanced standing students must reach 900 across both degrees.
Programs in this set structure the hours differently:
- UW-Whitewater: 480 field hours, usually in one semester at about 32 hours a week, or two semesters at 16 to 20 hours.
- Vermont State University: 400 supervised hours across two practicum courses.
- Indiana Wesleyan University (online): 400 supervised hours of field experience, arranged near you.
- UCF: a required internship in a community agency arranged by the school.
Ask each program which hospitals, hospices and home health agencies take its students. A semester-long block placement, like Whitewater’s, gives you enough continuous time on a unit to be trusted with real discharge cases.
Medical Social Worker vs Clinical Social Worker
The two titles describe different things. “Medical” names the setting: hospitals, clinics, hospice and home health. “Clinical” names the license and scope: BLS says clinical social workers “diagnose and treat mental, behavioral, and emotional problems” and provide individual, group, family and couples therapy.
| Medical social worker | Clinical social worker | |
|---|---|---|
| Focus | Coping with illness, discharge planning, resources | Diagnosing and treating mental health conditions |
| Typical settings | Hospitals, hospice, home health, nursing homes | Private practice, mental health centers, hospitals |
| Degree | BSW for hospice, nursing home and assistant roles; MSW for home health social worker | MSW required |
| License | Depends on the state and the role | Clinical license after supervised experience and an exam |
The two can overlap: a social worker in a medical setting can also hold a clinical license. If therapy interests you, compare this page with the mental health counselor career page.
How to Stand Out for Hospital Social Work Jobs
Use this checklist while you choose a program and plan your four years:
- Pick a health track. Florida Atlantic lists child welfare, addictions, aging and health tracks; UCF lists five, including Title IV-E and health.
- Plan the MSW now. A same-school route can save a year. UCF’s advanced standing MSW is 32 credit hours in 1 year.
- Ask for a health placement in writing. The federal hospice and nursing home rules both count health-care experience.
- Keep your GPA up. UCF’s limited-access major requires a 2.5 GPA to apply, and advanced standing programs set their own minimums.
- Get licensed early where you can. A bachelor’s credential lets you work in hospice or a nursing home while you finish the MSW part time.
Healthcare social workers earn a median $67,880, with the middle half between $56,710 and $82,240 (BLS, May 2025). For related routes, see case manager and school social worker, or browse all BSW careers.
Frequently Asked Questions
Can You Be a Medical Social Worker With Only a BSW?
In some settings, yes. Federal Medicare rules let a hospice hire a social worker with a CSWE-accredited BSW and 1 year of social work experience in a health-care setting, and nursing homes with more than 120 beds can count a bachelor's-level social worker with a year of supervised health-care experience. Home health agencies require a master's for the social worker title and use BSW graduates as social work assistants.
How Long Does It Take to Become a Medical Social Worker?
A BSW usually takes four years. With advanced standing, the MSW can take about one more year: Marshall University lists 30 or 36 hours in one calendar year, and Florida Gulf Coast University lists 1 year full time or 2 years part time. BLS notes that a standard MSW generally takes 2 years.
Is a Medical Social Worker the Same as a Healthcare Social Worker?
Medical social work is one specialty inside the federal 'healthcare social workers' occupation. BLS says some healthcare social workers specialize in geriatric social work, hospice and palliative care, or medical social work, and it reports their pay together.
Do Medical Social Workers Need a License?
Usually some form of license, set by your state. 42 of 52 jurisdictions issue a bachelor's-level social work credential, and all states license clinical social workers, which requires an MSW, supervised clinical experience and a clinical exam. California, Florida, Illinois and New York are among the 10 jurisdictions with no bachelor's-level license.
What Is the Most Affordable Way to Start?
Start at a public university in your state. Among the 14 programs with a health track on this page, federal tuition and fees for a full-time year (NCES, 2023-24) run from $4,879 at Florida Atlantic University, with a public-school median of $6,399 against $34,502 at private schools.
Can I Do the BSW Online?
Two of the 14 health-track programs on this page are online, including Indiana Wesleyan University's online BSW at $455 per credit hour. The 400 supervised field hours still happen in person at an agency near you.
